Is anything missing?

Walk through the whole plan together, then ask: “When you notice your warning signs, talk me through how you will use your plan.”

Listen for anything unclear or difficult. Clarify it together and check that the plan reflects what the person will actually do.

Confirm that the person has an accessible copy, including during telehealth or telephone care.

Check in next time

The next time you see the person, ask whether they needed the plan, whether they used it, and what got in the way. Keep reviewing and adding to the plan together.

Before finishing, ask Alex to look over his plan. “Is anything missing? Does this seem doable?” Walk through the whole plan together. Then ask, “When you notice your warning signs, talk me through how you will use your plan.” Listen for anything unclear or difficult, and clarify it together. Then ask, “On a scale of one to ten, how confident are you that you can use this plan?” Give him time to answer. If something seems difficult, talk it through together.

Confirm that Alex has an accessible copy, on his phone or written down, including when you are working by phone or telehealth. The goal is a plan Alex believes he can use. This is more than completing a card. It is helping Alex cope with distress and stay connected to the things he wants to live for, with practical steps he helped choose. The next time you see Alex, check in about the plan. “Did you use it? Was there a time you needed it but didn't use it?”

If so, ask, “What got in the way? Is there something we could change to make it easier to use?” Keep reviewing and adding to the plan together. Over time, Alex can build new coping skills and include what works for him.
